PROGRAM DESCRIPTION
Funded by the Community Partnership for the Prevention of Homelessness, the Community Care Grant Program is a time-limited, community based, family services program that assists families facing a housing crisis. Assistance is provided to obtain and/or maintain stable, safe and affordable permanent housing. Through a family case management plan, staff works with the family to resolve the problem(s) that led to their homelessness status. SERVICES PROVIDED
A. The client and family must be homeless within thirty days with a letter stating: Date to vacate; Number of children; Signatures and contact number of persons with whom client resides. B. The client takes the letter to The Virginia Williams Resource Center located at 25 M St., SW, where the Intake Screening Process begins. 1. A Case Manager interviews the client and a drug screening is administered. If the client fails the screening, the client will no longer be considered for the program. A referral will be made to a Drug Treatment Program. 2. Client must have Vital Statistics Papers for self and all dependents (Birth Certificates, social security cards, non driver's/drivers licenses, proof of income, school report cards, etc.). 3. If client was referred form a government or non-profit agency they must submit the referral to the case manager at the beginning of the intake process. 4. After all of intake requirements are completed, the case manager at the Virginia Williams Center contacts the Housing Coordinator at the Collaborative. FSFSC INTAKE PROCESS A. The Virginia Williams Resource Center Case Manager makes a referral to the Housing Coordinator who agrees to meet with the client. The Virginia Resource Center Case Manager provides the client with the Housing Coordinator's telephone number. The client calls the Housing Coordinator to schedule appointment. B. The client meets with the Housing Coordinator to ensure that he/she meets the criteria to receive housing assistance through the Options 2000 Program |
