Youth Aftercare

Mission
The mission of the Youth Aftercare Program (YAP), is to provide a community based support network in
Ward 8 for young adults that have or are going to exit the child welfare system. By providing supportive
case management services, community based employment, housing, educational opportunities, and
resource links; youths can successfully gain independence and bridge the gap between youth and
adulthood.

Eligibility
Youth, ages16-21 who are wards of the District of Columbia (in Foster Care, Transitional Housing, etc.)
that will emancipate (age-out) or be released within 6 months or less from the Child Welfare Systems.
Youth must be moving to or currently residing in Ward 8 of Southeast, Washington, DC.
